We know filing a claim can be an upsetting and stressful process. We’re dedicated to making it as easy for you as possible, and will be with you until everything is resolved.

Reporting a Claim

We make it simple. To report a claim, give us a call at 800-225-1865 and a dedicated Fred C. Church claims representative will be happy to assist you. Following your report, we will remain involved in order to monitor your claim and provide updates on a regular basis.  Call us Monday through Friday from 8:30am to 5:00pm.

Claims FAQs

What happens if I have a claim over the weekend or when your offices are closed?

You can call our office and leave a message in our general mailbox and a claims representative will return your call the next business day.  Or, you can wait until the next business day and call us.  If the claim is of a serious nature (i.e. personal injury involved), you may call our answering service who will contact the appropriate Fred C. Church representative.  The answering service can be reached by dialing 800-225-1865 and pressing 2 when prompted.


Vehicle Claims

You will need the following information from the driver of the other vehicle:

  • Owner Name
  • Address
  • Phone #
  • Name of Operator of Other Vehicle
  • Driver's License #
  • Plate Registration # and State
  • Vehicle Year, Make and Model
  • Name of Insurance Company
